I acknowledge that I have voluntarily requested prayer ministry from members of the Hope Global Freedom Prayer team. This may include various inner healing techniques and, where appropriate, pastoral care. I understand that this is not a professional counseling service and that the individuals ministering to me are not licensed counselors or therapists. However, I recognize that they are offering support to the best of their training and ability with the intention of helping me experience greater freedom in my life.
I understand that Hope Global is a non-profit organization and does not charge for its ministry services. I affirm that I am seeking this help of my own free will and that I'm under no obligation to follow or accept any advice or direction offered by team members. Hope Global offers biblical spiritual support to anyone regardless of financial circumstances. While there is a suggested donation of $20 per session, any contribution is voluntary and appreciated as it helps support our pastoral team. Receipts are available upon request.

I understand that if I receive ministry from the Freedom Prayer Team, any personal information shared will be treated with respect and sensitivity. However, complete confidentiality is not guaranteed, and relevant information may be shared.
I also understand that Hope Global is legally required to report the following:
- Any expressed intent to harm oneself or others; or to carry out a dangerous or criminal act.
- Any known or suspected cases of current child or elder abuse or neglect.
By signing this, I release Hope Global's team members & affiliates from any liability, loss, or damage that may arise from the support or ministry I receive.
